- Personal Information. By submitting this survey, you consent to us collecting, storing, and using your personal information and survey responses for customer service, product recommendations, future reference, marketing communications, promotional offers, and business improvement, in accordance with our Privacy Policy.
- Introductory Offer. Upon completing the survey and paying the $12 shipping contribution, you will receive one complimentary wellness supplement selected based on the gender you provide in your survey.
- Subscription. Participation in this offer automatically enrols you in our subscription program, which includes a minimum three-month commitment. Your subscription pricing is as follows:
- Month 1: Complimentary product — pay only the $12 shipping contribution.
- Month 2: Receive 50% off the standard retail price.
- Month 3 and ongoing: Receive 25% off the standard retail price.
- Automatic Billing. Your nominated payment method will be automatically charged each month on or around the same date you joined the subscription, unless cancelled after the minimum three-month commitment.
- Cancellation. The subscription cannot be cancelled during the initial three-month commitment. After this period, you may cancel at any time before your next scheduled billing date to avoid future charges.
- Marketing Communications. By participating, you consent to receive marketing emails, SMS messages, newsletters, and promotional offers. You may unsubscribe from marketing communications at any time; however, unsubscribing does not cancel your subscription.
- General. We reserve the right to amend these Terms and Conditions where reasonably necessary. Nothing in these Terms limits your rights under the Australian Consumer Law.
